糖尿病患者医院社区连续性健康教育效果评价

摘    要:目的:探讨糖尿病患者医院社区连续性健康教育的效果,提高患者的生活质量。方法:将126例糖尿病患者随机分为干预组(63例)与对照组(63例),对照组仅在住院时进行糖尿病知识宣教,干预组实行双向转诊管理,根据患者的具体情况进行健康干预,使其掌握正确的用药方法,合理控制饮食,按时监测血糖,保持适当运动,预防并发症等,提高患者对治疗的依从性。结果:与对照组相比,干预组患者自我管理能力提高,遵医行为明显改观,门诊平均医疗费用降低,并发症的发生减少。结论:双向转诊管理实施连续性健康教育有利于提高糖尿病患者的认知水平和自我管理能力,减少门诊平均医疗费用,提高患者生活质量。

Abstract:Objective:To investigate the effect of continuous health education for diabetic patients in hospitals and communities and improve quality of patients' life.Methods:One hundred and twenty-six patients were randomly divided into intervention group(63 cases)and control group(63 cases).Patients in control group received diabetic knowledge education only when they were in hospital,while patients in intervention group were under two-way referral management and health intervention based on their conditions,which can help patients to master correct medication,reasonably control diet,monitor blood glucose in time,keep adequate physical exercise,prevent complication,then raised patients'compliance to treatment.Results:Compared with control group,patients' self-management ability in intervention group incresed,their medical compliance behavior was markedly improved,besides,average cost in outpatient decresased and complication incidence declined.Conclusion:Two-way referral management and continuous health education can boost diabetic patients' cognity level and self-management ability,decrease average cost in outpatient,and improve patients' life quality.
