急性胰腺炎82例的CT诊断

目的：探讨急性胰腺炎（AP）的CT表现及其诊断价值。方法：回顾性分析82例临床诊断的AP病例的临床及CT资料。结果：82急性胰腺炎中,水肿型61例,坏死型21例。CT表现按出现率的高低依次为：胰腺肿胀、两侧或左侧肾前筋膜增厚及肠道淤张、胰腺边缘模糊、胆囊增大胆总管扩张、胰周蜂窝组织炎、腹水。结论：CT能判断AP类型及分级,估计胰腺的肿大、坏死程度、胰液外渗量和范围、有无并发症,是临床判断病情、制定合理治疗方案及准确评估预后的重要依据。

CT diagnosis of 82 acute pancreatitis cases

Objective:To investigate the CT manifestations of acute pancreatitis and its diagnostic value. Methods: Clinical and CT data of 82 patients who were diagnosed as acute pancreatitis were retrospective analyzed. Results: Of the 82 acute pancreatitis cases,61 were categorized as edema type,21 as necrosis type. A CT manifestation list in order of frequency were: pancreatic swelling,either side or the left side of renal fascia thickening and intestinal tract bloating,blurred pancreas edge,expanded gallbladder and dilated choledochal duct,cellulitis surround the pancreatic and ascites. Conclusion: Type and grade of acute pancreatitis can be shown by CT scan. Enlargement and necrosis of pancreatic,volume and invoved area of pancreatic juice extravasation,whether or not there are complications can also be judged under CT. CT manifestations can be used as the basis for the treatment and prognosis of accurate pancreatitis.
