| 摘 要： | 目的：观察双水平无创正压通气对慢性阻塞性肺病（COPD）并发呼吸衰竭患者的治疗作用。方法：选择34例COPD伴Ⅱ型呼吸衰竭患者在常规治疗的同时，用双水平正压呼吸机（BiPAP）进行无创通气治疗，观察治疗前、后的临床及动脉血气变化。结果：28例患者经机械通气治疗后意识恢复正常，血氧饱和度（SaO2）逐渐升高，二氧化碳分压（PaCO2）明显下降（P〈0．05）；2例放弃治疗，4例死亡。结论：BiPAP对COPD并呼吸衰竭疗效肯定，无严重不良反应。

| Abstract: | Objective: To observe the effect of noninvasive positive pressure ventilation in the treatment of chronic obstructive lung disease(COPD) with respiratory failure.Methods: 34 COPD patients with respiratory failure were treated by BiPAP noninvasive positive pressure ventilation as taking routine treatment.Clinical symptoms and arterial blood gas were observed before and after treatment.Results: 28 patients got back to normal consciousness,with gradually increasing SaO2 and significantly declining PaCO2(P<0.05).2 patients abandoned the treatment,and 4 patients died.Conclusion: BiPAP has confirmed therapeutic effect on chronic obstructive lung disease(COPD) with respiratory failure,and has no adverse effect. |
