低温等离子消融技术在功能性腭咽成形术中的应用研究

摘    要:【目的】探讨低温等离子消融技术在功能性腭咽成形术治疗阻塞性睡眠呼吸暂停低通气综合征(obstructivesleepapneahypopneasyndrome,OSAHS)中的应用价值。【方法】在低温等离子消融技术辅助下采用功能性腭咽成形术治疗多平面阻塞OSAHS患者58例,今部随访半年以上,术后6个月复查多导睡眠监测、纤维喉镜,与术前数据比较,判定疗效。【结果】58例患者中,全部病例均未出现严重并发症。AHI由术前(65.23±21.27)下降至(27.82±18.45)(P〈0.01);LSpO2由术前(76.40±15.30)17.升至(83.73±9.68)(P〈0.05),总有效率为84.5%(49/58)。【结论】功能性腭咽成形术治疗OSAHS效果良好,低温等离子消融技术有助于该手术疗效的提高,具有安全、微创、简便易行等优点。

Abstract:Objective]To investigate the value of plasma radio-frequency ablation in functional uvulopalatopharyngoplasty(UPPP) for patients with obstructive sleep apnea-hypopnea syndrome( OSAHS). Methods] Using polysomnography and fiberoptic pharyngocopy evaluate the effect of the functional uvulopalatopharyngoplasty. Results] The average apnea-hypopnea index reduced from 65.23 ±21.27 to 27.82 ±18.45 (P 〈 0.01 ) and the lowest pulse oxygen saturation (LSpO2) was improved from 76.40± 15.30 to 83.73 ± 9.68 (P 〈 0.05 ). The total effective rate was 84.5 % (49/58). AHI and SaO2 appeared an obvious difference between the before surgery and after surgery. Conclusion] Functional UPPP is a safe and efficient treatment for OSAHS. Plasma radio-frequency ablation can facilitate the improvement of therapeutic effect on functional UPPP for this disease.
