糖尿病周围神经病变危险因素分析

目的探讨糖尿病周围神经病变(DPN)的相关危险因素。方法收集254例2型糖尿病患者的年龄、体质指数、腰臀比、糖尿病病程、血压、空腹血糖、餐后2h血糖、糖化血红蛋白、血脂、胰岛B细胞功能等临床资料。根据神经传导速度(NCV)将患者分为DPN组(144例)和非DPN组(110例)。对有统计学意义的危险因素行多因素Logistic回归分析,筛选DPN的危险因素。结果DPN组的年龄、糖尿病病程、收缩压、糖化血红蛋白水平均显著高于非DPN组(P值均<0.01)。多因素Logistic回归分析提示,年龄(OR=1.051,95%CI为1.018~1.085,P=0.002)、糖尿病病程(OR=1.059,95%CI为1.008~1.113,P=0.022)和糖化血红蛋白(OR=1.183,95%CI为1.060~1.319,P=0.003)是DPN的危险因素。结论DPN的发生与患者的年龄、糖尿病病程和糖化血红蛋白增高有关,代谢因素在其发病机制中起重要作用,是DPN的独立危险因素。

Analysis on risk factors for diabetic peripheral neuropathy

Objective To study the risk factors for diabetic peripheral neuropathy (DPN). Methods The clinical data of 254 patients with type 2 diabetes, including the age, body mass index (BMI), waist/hip radio (WHR), course of diabetes, blood pressure, fasting plasma glucose (FPG), 2 h postprandial plasma glucose (2 hPG), HbA1c, lipid and islet B cell function were collected. The patients were divided into 2 groups according to the nerve conduction velocity (NCV): DPN group (144 cases) and non-DPN group (110 cases). The risk factors of statistical significance were subjected to multiple logistic regression analysis to screen for the risk factors of DPN. Results The ages, courses of diabetes, systolic pressures and HbA1c in the DPN group than were significantly higher than those in the non-DPN group (all P<0.01). Multiple logistic regression analysis revealed that DPN was correlated with patients' age (OR=1.051, 95%CI=1.018-1.085, P=0.002), course of diabetes (OR=1.059, 95%CI=1.008-1.113, P=0.022) and HbA1c (OR=1.183, 95%CI=1.060-1.319, P=0.003). Conclusion DPN is associated with the increase of age, courses of diabetes and HbA1c. Metabolic factor is important in DPN pathogenesis and is an independent risk factor for DPN.
