影响骨巨细胞瘤预后的临床病理相关因素分析

目的研究影响骨巨细胞瘤预后的临床病理因素。方法对本院诊治的56例骨巨细胞瘤病例进行追踪随访，并对年龄、性别、发病部位、肿瘤大小、病理骨折、Campanicci’s影像学分级、WHO病理新分类及手术方式等临床病理相关因素与局部复发的关系进行统计分析。结果56例骨巨细胞瘤中9例局部复发，复发率为16．1％。恶性巨细胞瘤术后复发率（100％）和Campanicci’s Ⅲ级复发率（31．8％）较对照组高。而4种手术方式治疗中，单纯刮除术后局部复发率最高，为35％。肺部转移3例，均为恶性巨细胞瘤。结论WHO病理新分类、Campanicci’s分级和外科手术方式与骨巨细胞瘤复发具有相关性。

Analysis of the factors affecting the prognosis of human giant cell tumors of bone

Objective To investigate the clinicopathologic factors affecting the prognosis of human giant cell tumor of bone. Methods There 56 cases with giant cell tumor of bone were followed up. The relationship of recurrence with age, sex, location of tumor, tumor size, pathological fracture, Campanicci ＇ s grading, pathological grading and operative modes was analyzed. Results Recurrence was observed in 9 of the 56 cases with a recurrent rate of 16.1%. Recurrence rates of malignancy in giant cell tumor group （ 100% ） and Campanicci＇ s grading Ⅲ group （31.8%） were higher than that of control group. The recurrrent rate in intralesional curettage group was the highest （35%） am ong the four different operative modes groups. Lung metastasis occurred to 3 malignant cases. Conclusion Campanicci＇ s grading, pathological grading and operative modes are associated with the recurrence of giant cell tumor of bone.
