ANCA及AECA在结节性红斑的诊断价值

目的探讨血清抗中性粒细胞胞浆抗体（ANCA）及抗血管内皮细胞抗体（AECA）在结节性红斑发病中的作用和意义。方法收集结节性红斑患者和正常人血清，用间接免疫荧光技术检测血清中ANCA和AECA，并分型。结果32例结节性红斑患者ANCA阳性率为12．5％，均为核周型ANCA（P—ANCA），与正常对照组比较差异无统计学意义（P〉0．05）。AECA阳性率为81．3％，其中IgG—ANCA阳性率为71．9％，IgM—AECA阳性率53．1％，与正常组对照组比较有显著性差异（均P〈0．01）。在发生结节性红斑及血沉增快方面AECA阳性组高于阴性组（P〈0．01）；而在伴有口腔溃疡和贫血等临床表现方面ANCA及AECA阳性组与阴性组比较差异均无统计学意义（P〉0．05）。结论结节性红斑患者血清AECA测定对该病血管损害及病情活动具有一定意义。

Value of serum anti-nuotrophil cytoplasmic autoantibodies and anti-vascular endothelial cell antibodies in diagnosis of erythema nodosum

Objective To discuss the value of detection of serum serum anti - nuotrophil cytoplasmic autoantibodies （ AN- CA） and anti - vascular endothelial cell antibodies （AECA） in the diagnosis of erythema nodosum. Methods ＇ Samples from erythema nodosum patients and normal people were collected, the ANCA and AECA were detected by indirect immunofluorescence. Results The positive rate of ANCA in 32 erythema nodosum patients was 12.5%, all Peri - ANCA. without showing significant differences as commpare to the normal controls （P 〉 0.05 ）. AECA positive rate was 81.3%, IgG- ANCA positive rate was 71.9%, IgM - AECA positive rate was 53.1%, showing significant differences as compared to that of the controls （ P 〈 0.01 ）. In the event of erythema nodosum and ESR by fast AECA positive aspects than negative group （P 〈 0. 01 ） ; There were no significant differences were observed between the two groups in oral ulcers, anemia,etc （P 〉 0.05）. Conclusion Detection the serum AECA is of certain value in disclosing the vascular lesions and the changes of the disease.
