大黄素对重症急性胰腺炎模型大鼠肺损伤的早期保护作用及其机制

摘    要:目的]探讨大黄素对大鼠重症急性胰腺炎(SAP)肺损伤的早期保护作用及其机制.方法]将SD大鼠随机分为假手术组、SAP组及大黄素组,再将每组分为术后4,8,12h亚组,检测各组血浆淀粉酶、肺泡灌洗液磷脂酶A2(PLA2),白细胞介素(IL)-18及IL-6水平,并对肺组织进行病理评分,采用RT—PCR法测定肺组织表面蛋白A(SP—A)mRNA表达,且与细胞因子水平进行相关性分析.结果]大黄素组血浆淀粉酶、肺泡灌洗液PLA2水平及肺组织病理评分与同时间点SAP组相比较均明显降低(P〈0.05);大黄素组肺泡灌洗液IL-1β,IL-6值与4h时SAP组相比较明显降低(P〈0.05);大黄素组肺组织SP-AmRNA表达水平明显高于SAP组(P〈0.01);肺组织SP—AmRNA表达在4,8h与IL-18,IL-6呈负相关(r=-0.587,P〈0.05).结论]大黄素通过抑制PLA2活性及降低炎性因子,减轻SAP模型早期肺炎症反应,提示大黄素有良好的肺组织保护作用,改善SAP所致的早期肺损伤.

Abstract:OBJECTIVE To study the early protective effects and its mechanism of emodm on lung injury in SAP model of rats. METHODS SD rats were randomly divided into sham operation group, SAP group and emodin group, and each group was further divided into postoperative 4 h, 8 h and 12 h subgroups, and the plasma amylase and phospholipase A2(PLA2), IL-1 ;, IL-6 content of bronchoalveolar lavage fluid (BALF) were measured and pathological score of lungs was graded. The expression of surfactant protein A (SP-A)of the lungs was detected by RT-PCR, and the correlation analysis was performed with cytokine levels. RESULTS The plasma amylase, PLA2 of BALF and lung pathology score at the same point in time decreased obviously in emodin group than in group SAP (P〈0.05), and IL-1 ; and IL-6 of BALF at 4 h decreased significantly in emodin group than in group SAP (P〈0.05). The expression of SP-A mRNA of the lungs increased significantly in emodin group than in group SAP, and the expression between SP-A mRNA and IL-1 ;, IL-6 in the lungs were negatively correlated(r = -0. 587, P%0.05). CONCLUSION Emodin can alleviate early inflammation of lungs in SAP model of rats by inhibiting the activity of PLA2 and reducing the inflammatory factors, and it is suggested that emodin has good protective effect to lung, and improve early lung injury induced by SAP.
