16-MSCT对视神经管微细或隐匿性骨折的诊断价值

摘    要:目的:探讨16层螺旋CT(16-MSCT)对外伤性视神经管微细或隐匿性骨折(micro-or occult-fracture’s,MOF’s)的诊断价值。方法:对常规头颅CT检查发现视神经管骨折或可疑骨折者,复查时常规5 mm层厚螺旋扫描后进行0.625、1.25、2.5及5 mm高分辨率骨算法重建(以下简称骨重建),收集数据完整者45例,结合二维及三维图像进行阅片;按不同层厚计算MOF’s的显示分值及显示率,进行自身对照。结果:45例均查出有MOF’s,每例1~11处不等。不同层厚下MOF’s的显示率:0.625 mm层厚为100%,1.25 mm层厚为95.35%,2.5 mm层厚为75.19%,5 mm层厚为41.09%;不同层厚下MOF’s的显示率比较差异均有统计学意义(P<0.05);重建层厚与显示率之间存在线性趋势。结论:16-MSCT趋于各向同性,在本研究范围内,重建层厚越薄,MOF’s显示越清楚。对可疑视神经管MOF’s的病例应常规作0.625 mm骨重建,以便及时准确地提供诊断信息,以降低视力损伤以及致盲的发生率。

Abstract:Objective: To evaluate the value of 16-MSCT in diagnosis of traumatic optic canal(micro-or occult-fracture′s,MOF′s).Methods: The paitients with the suspected optic canal micro-or occult-fracture′s(MOF′s)were followed up.They were adopted the conventional 5 mm thin-section helical CT scaning,then carries on 0.625,1.25,2.5 and 5 mm high resolution bone algorithm reconstruction separately in the reexamination.There were 45 cases as a sample and the union two-dimensional and the three-dimensional images were combined to calculate the value and the detection rate according to the different thin-section MOF′s demonstration,then own comparison separately was carried out.Results: MOF′s were founed in 45 patients,each had 1-11 fractures.MOF′s detection rate: the 0.625 mm thin-section was 100%,the 1.25 mm was 95.35%,the 2.5 mm was 75.19%,the 5 mm was 41.09%,There were significance among them(P<0.05).Conclusions: 16-MSCT tends an isotropism.In this studies,the section of reconstructs is thinner,the MOF′s shows more clearer.It is necessary that the patients with suspected optic canal MOF′s carries on 0.625 mm high resolution bone algorithm reconstruction,so as to providing eimely and accurate diagnosis information and reducing the incidence of blindness and eyesight damage.
