新生儿轻、中度缺血缺氧性脑病终期的MRI表现

目的：探讨新生儿轻、中度缺血缺氧性脑病终期的MRI表现及其诊断价值。方法：对51例围产期有轻、中度缺血缺氧病史，临床上表现为脑瘫的早产或足月产忠儿进行MRI研究。结果：51例中的19例早产儿MRI表现为脑室周围脑白质软化症；32例足月儿中25例MRI表现为分水岭区脑损伤，7例表现为脑室周围脑白质软化症。结论：早产及足月新生儿轻、中度缺血缺氧性脑病的终期均具有典型的MRI表现，MRI可作为显示其病理改变的首选影像学检查方法。

MRI findings of mild to moderate neonatal hypoxia-ischemia encephalopathy in end stage

Objective: To evaluate the MRI findings and their diagnostic values in mild to moderate neonatal hypoxia-ischemia encephalopathy at end stage. Methods: MRI studies were performed in 51 patients aged from 8 months to 13 years, who suffered from cerebral palsy with histories of either premature or term delivery, and mild to moderate perinatal hypoxia or ischemia. Results: Two patterns of end-stage cerebral injury were demonstrated on MR images depending on the gestational age when the anoxic episode occurred. In 19 cases of premature anoxia, MRI demonstrated periventricular leukomalacia (PVL). In 25 of 32 cases of term asphyxia, MRI showed watershed injuries. Another 7 cases showed PVL, the same as findings of premature anoxia. Conclusion: MRI performed at end stage in either premature or term infants with perinatal hypoxia-ischemia brain damage is sensitive and specific in demonstrating the pathological changes, and it can also be used as the choice in diagnosing this disease.
