神经系统自身免疫疾病患者泪液的细胞学研究

研究１５例神经系统自身免疫疾病急性期的泪液细胞学，其中重症肌无力５例，急性脊髓炎４例，多发性肌炎２例，急性感染性多发性神经炎２例，多发性硬化１例，视神经脊髓炎１例，并以９例正常人泪液的细胞学作对照。结果表明，实验组泪液细胞阳性率明显高于对照组（Ｐ＝０．０１），粒细胞和淋巴细胞反应两组间差异有显著性，（Ｐ＝０．０００６，Ｐ＝０．０１４），单核细胞两组间无明显差异（Ｐ＝０．０５）。实验组泪液中有激活淋巴细胞、大淋巴样细胞、浆细胞、激活的单核样细胞，并有明显的粒细胞反应。提示：病毒感染引起的粘膜免疫反应可能是导致神经系统自身免疫病理过程的重要原因。

STUDY OF THE LACHRYMAL FLUID CYTOLOGY IN PATIENTS WITH NEUROSYSTEMIC AUTOIMMUNITY DISEASES

Lachrymal fluid cytology was studied in 15 patients with neurosystemic autoimmunity diseases when they were in acute period and had not been treated systemically. The patients included 7 men and 8 women, ranging in age from 12 to 68. The diseases included 5 cases of myasthenia gravise, 4 of acute myelitis, 2 of polymyositis, 2 of Guillain-Barre Syndrome, 1 of multiple sclerosis, 1 of Devic disease. Meanwhile, lachrymal fluid cytology in 9 normal persons was examined as a control. Statistical results showed that the positive rate of lachymal fluid cytology in Patients was significantly higher than that in the normal (P=0.01). Significant differences were found between granular leukocyte response in the two groups(P=0.0006). The same result was also found in lymphocyte response (P=0.01). Significant differences were not found between monocyte response in the two groups (P=0.05). In the lachymal fluidof patients, activated lymphcytes, large lymphoid cells, piasma cells and activated monocytoid cells were observed, and there Was granular leukocyte response. The results of the study indicate that virus infection and immune response of mucosa exist in neurosystemic autoimmunity diseases. The occurrence,recurrence and deterioration of the disease may be related to virus infection of mucosa. The immune response of mucosa caused by virus infection may be the important reason for the pathological process of neurosystemic autoimmunity.
