摘    要:目的 探讨原发输尿管癌的影像学诊断的方法,评价临床价值.方法 收集近5年来,经手术治疗过的原发输尿管癌46例,均经MR检查.所有病例先行冠状面重T2WI水成像所显示输尿管积水,如有梗阻,在梗阻水平T2WI和T1WI扫描,并作进一步动态增强扫描.术前对各种影像诊断的报告比较分析.结果 在46例中,有42例患侧输尿管有不同...

Abstract:Objective To investigate examination methods and imaging manifestations of primary ureteral carcinoma,and to evaluate its clinical values.Methods 46 cases of primary urelteral carcinoma,who were operated within recent 5 years,were studied.All cases were performed MRI examination.Coronal heavy T2WI(water imging) was first performed to show the dilated ureter.If there is any obstruction,then axial T2WI and T1WI were scanned at the obstruction level,and underwent additional Gd-DTPA dynamic contrast enhanced scans.The diagnostic reports of various imaging facilities were analyzed comparing with the rusults of operation and pathology.Results MRI showed the ureteral dilatation of the different degrees in 42 of 46 cases,no abnormal appearance in 1 case.Among the 38 cases with ureteral obstruction,soft mass at the obstruction level was detected on axial scans.The lesions showed gradual and homogeneous mild to moderate enhancement on contrast MRI.The accurate diagnostic rate before the operation was as follows:MRI(100%),RUP(76%),CT(65.0%),ultrasound(56.52%),IVU(10.87%).Conclusion Combination of MR water imaging and conventional sequences can demonstrate most primary ureteral carcinoma lesions and has a highest diagnostic accuracy among the current diagnostic imaging facilities.
