过敏性紫癜患儿T细胞亚群与免疫球蛋白水平检测及分析

目的　观察过敏性紫癜 (HSP)患儿T细胞亚群与免疫球蛋白的动态变化 ,以探讨其发病原理及治疗对策。方法　分别采用流式细胞技术和散色比浊法测定HSP急性期与恢复期及对照组儿童血清免疫球蛋白浓度和T淋巴细胞亚群水平。结果　HSP患儿急性期IgA浓度明显高于对照组 ,恢复期与正常对照组无显著性差异 ;急性期C3 明显降低 ,恢复期正常。急性期CD3 、CD4 含量、CD4 /CD8比值及NK细胞均低于恢复期组(P <0 0 1、P <0 0 5、P <0 0 1、P <0 0 1) ,而恢复期组又低于对照组 (P <0 0 5、P <0 0 5、P <0 0 1、P <0 0 1)。结论　HSP患儿在体液免疫与细胞免疫方面均有异常改变 ,且与病情相关。研究提示 :在常规治疗HSP时可加用免疫调节剂

Detection and Analysis of Blood T-cell Subgroup and Immunoglobul- in Level of Henoch-Schonlein Purpura of Children

Objective To observe the dynamic changes of T-cell subgroup and immunoglobulin level of Henoch-Schonlein purpura (HSP) of children so that to discuss the mechanism and its treatment. Methods The immunoglobulin concentration and T-cell subgroup level of acute stage and convalescent stage HSP children were detected with FCM and nephelometer. Results IgA concentration in acute stage of HSP children was significant higher than that in control group. IgA concentration of convalescent stage of HSP children was no significant difference with that in normal group. C3 in acute stage decreased obviously and than turn to normal in convalescent stage. The contents of CD3 and CD4, ratio of CD4/CD8 and NK cell were lower than those in convalescent stage (p<0.01, p>0.05, p<0.01, p<0.01) and those in convalescent stage were lower than in control group (p<0.05, p<0.05, p<0.01, p>0.01). Conclusion There were abnormal changes in body fluid immunity and cell immunity in HSP and related with state of illness. The study demonstrated that the immune regulator could be used in the routine therapy of HSP.
