恶性胆管梗阻的低场磁共振胆管成像诊断

目的　评价低场磁共振胆管成像 (MRCP)对恶性胆管梗阻性疾病的诊断价值。方法　对 6 7例临床疑诊恶性胆管梗阻的患者进行了MRCP检查 ,并与ERCP/PTC和手术病理结果相对照。结果　对恶性胆管梗阻MRCP总的诊断价值为敏感度 86 5 7% ,特异度 10 0 % ;ERCP/PTC总的诊断价值为敏感度 91 0 4 % ,特异度 10 0 %。两者相比 ,统计学无显著性差异 (χ2 =0 4 7,P >0 0 5 )。MRCP对恶性胆管梗阻的定位诊断准确率为 10 0 % ,定性诊断准确率为 91 2 8%。结论　低场磁共振MRCP检查成功率高 ,对恶性胆管梗阻的定位、定性准确 ,可为外科手术或临床保守治疗提供客观依据。

Low Field MR Cholangiopancreatography in Diagnosis of Malignant Bile Duct Obstruction

Objective To evaluate the value of magnetic resonance cholangiopancreatography of low field MR scanner in diagnosis of malignant biliary obstructive disease. Methods 67 patients with suspected malignant biliary obstructive disease were examined by MRCP. The results of MRCP were compared with that of ERCP or PTC and pathology. Results For the disease, the sensitivety and specificity of MRCP and ERCP/PTC were 86.57%, 100% and 91.04%, 100%, there was no significant difference between MRCP and ERCP. The accuracy of MRCPin detection of the degree and level of malignant biliary obstruction was 100% and the accuracy for evaluating the causes of obstruction was 91.28%. Conclusion Low field MRCP examination might have a high successful rate and accuracy define the level and cause of malignant bile duct obstruction. It could be helpful to provide useful information for surgical therapy and palliative drainage procedures.
