腹部闭合性损伤临床诊断与治疗中超声的应用价值

目的探讨与研究急性腹部闭合性损伤超声诊断对临床选择治疗方法的价值。方法回顾并分析我院自2008-02～2011-01期间,急诊超声诊断腹部闭合性损伤226例患者的临床选择治疗的价值。结果 226例患者超声诊断腹部闭合性损伤,根据超声图像的特点,挫伤绝大部分选择非手术治疗;血肿、实质性损伤大部分选择手术治疗;而破裂伤的患者全部选择手术治疗。本组选择手术治疗的患者为158例,占总患者数的69.91%;选择非手术治疗的患者68例,占30.08%。158例患者经过手术后证实均有不同脏器腹部闭合性损伤,68例非手术患者中,有4例为延迟性出血,除这4例患者外,均采取保守治疗并全部痊愈出院。结论超声在外伤诊断中无损伤、无痛苦,给患者带来的伤害较小,并可有效明确腹部闭合性损伤脏器的部位,以及患者腹腔的出血量等。超声诊断腹部闭合性损伤,具有快速、准确等特点,是临床选择治疗方法的可靠依据,值得在临床上进行推广并应用。

Research on the Value of Ultrasonography in Blunt Abdominal Trauma Clinical Diagnosis and Treatment

Objective To investigate ultrasound diagnostic value in the acute abdominal closed trauma.Methods To review and analyze the value of the emergency ultrasound diagnosis of blunt abdominal trauma in 226 patients during 2008-02 to 2011-01Results 226 cases of ultrasound diagnosed of abdominal closed trauma were studied.According to the characteristics of ultrasound images,the vast majority contusions were choose non-surgical treatment,most of hematoma,parenchymal damage were choose surgery,while rupture in patients choose surgery.158 cases of 69.91% the total treated with surgery,non-surgical treatment of 68 patients,accounting for 30.08%.158 cases were confirmed have varying abdominal closed trauma after surgery,4 cases out of 68 patients had delayed bleeding,in addition to these 4 patients,all treated conservatively and discharged.Conclusion Ultrasound in the diagnosis of trauma have non-injury,no pain,less damage caused to patients,the more effectively clear abdominal closed trauma parts,and peritoneal bleeding etc.Ultrasonic diagnosis of abdominal closed trauma with fast and accurate characteristics,are reliable basis for selecting treatment methods,have worthy of promotion and application in clinical practice.
