人工机械瓣膜与生物瓣膜在二尖瓣置换术中的疗效观察

摘    要:目的探讨人工机械瓣膜与生物瓣膜在二尖瓣置换术(MVR)中的疗效。方法以广东省高州市人民医院2016年3月至2017年8月收治的72例二尖瓣置换术患者为研究对象,随机分为对照组(36例)和观察组(36例);对照组采用人工机械瓣膜,观察组采用生物瓣膜;比较两组患者疗效及安全性。结果观察组手术时间(80.3±21.2) min]与住院时间(8.4±1.1) d]明显短于对照组(122.5±20.7) min,(10.6±2.3) d](P<0.05);术后2周,两组各项心功能指标均明显好转,且两组术前与术后各指标比较,差异无统计学意义(P>0.05);术后18个月,观察组治疗有效率(97.2%)高于对照组(86.1%),但差异无统计学意义(P>0.05);术后18个月,观察组并发症发生率(0.0%)明显少于对照组(16.7%)。结论生物瓣膜在二尖瓣置换术中能够明显缩短手术时间及住院时间,且生物瓣膜置换术后患者各项心功能指标较好,治疗有效率高,并发症少,适合在临床推广应用。

Abstract:【Objective】To investigate the effect of artificial mechanical valve and biological valve in mitral valve replacement.【Methods】In our hospital from March 2016 to August 2017,72 patients with mitral valve replacement were randomly divided into the control group(36 cases)and the observation group(36 cases).The control group were treated with artificial mechanical valve,and the observation group were treated with biological valve.The efficacy and safety of the two groups of patients were compared and analyzed.【Results】The operation time(80.3±21.2 min)and hospitalization time(8.4±1.1 d)in the observation group were significantly shorter than those in the control group(122.5±20.7 min,10.6±2.3 d)(P<0.05).The cardiac function indexes were significantly improved two weeks after operation,and there was no significant difference between the two groups before and after operation(P>0.05).After 18 months,the effective rate of the observation group(97.2%)was higher than that of the control group(86.1%),however,the difference was not statistically significant(P>0.05);18 months after surgery,the incidence of complications in the observation group(0.0%)was significantly lower than that in the control group(16.7%).【Conclusion】The biological valve can significantly shorten the operation time and hospitalization time in mitral valve replacement,and the patients have better cardiac function indexes after bioprosthetic replacement.Biological valve replacement has high treatment efficiency and less complications,which is suitable for clinical application.
