剖宫产瘢痕妊娠不同处理方式的分析

目的对剖宫产瘢痕妊娠的不同处理方式进行评价。方法对该院2000年4月～2003年3月收治的5例(A组)及2006年1月～2007年6月收治的20例(B组)剖宫产瘢痕妊娠处理方式进行回顾性分析。A组患者进行超声监测下清宫术,B组患者进行超声监测下宫腔镜病灶切除术。结果B组平均孕周大于A组;B组患者术中出血量少,术后血人绒毛膜促性腺激素(hCG)下降迅速,局部包块吸收快。结论宫腔镜下病灶切除术认为是目前治疗剖宫产瘢痕妊娠的有效措施,它具有术中出血少,病灶切除确切,相对较安全,术后血hCG下降迅速,局部包块吸收快,无须2次手术的特点。

Analysis of different managements of cesarean scar pregnancy

Objective] To evaluate different managements of cesarean scar pregnancy.Methods] 25 cases of pregnancy on cesarean scar were reviewed retrospectively.The patients were divided into two groups:5 cases admitted uterine suction from April 2000 to March 2003 in Group A and 20 cases admitted hysteroscopic resection from January 2006 to June 2007 in Group B.All patients were monitored by ultrasound during operation.Results] Patients in group B had longer gestational weeks,less blood loss,faster drop in HCG level and quicker mass absorption.Conclusions] Hysteroscopic resection is regarded as the effective management of cesarean scar pregnancy.No second operation is needed after hysteroscopic removal of cesarean scar pregnancy.
