超声心动图评价房间隔缺损封堵术后左室功能变化

摘    要:目的 应用超声心动图评价左室功能在房间隔缺损(ASD)封堵治疗术后与随访中的变化。方法 对该院成功实施经导管ASD封堵术且资料完整的36例患者进行研究。所有患者均于封堵治疗术前、术后3d、术后3个月及术后6个月行超声心动图检查,测量左室舒张末期内径(LVEDD)、左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)、左室每搏量(Lvsv)、左室射血分数(LvEF)和左室短轴缩短率(LVFS)。结果 ASD封堵治疗术后,LVEDD和LVEDV明显增大,提示左室前负荷改善;LVESV轻度增加;LVSV、LVEF和LVFS显著增加,表明左室功能增强。LVESV在术后3个月和术后6个月随访时与术后3d比较无明显变化,左室前负荷和左室功能在随访中持续改善。结论 ASD封堵治疗术能有效改善左室前负荷,改善左室功能,且在短、中期随访中左室前负荷及左室功能持续改善。

Abstract:Objective To evaluate the changes of left ventricular function after transcatheter closure of atrial septal defect (ASD) and during a short to mid-term follow-up by echocardiography. Methods Thirty-six patients with secundum ASD underwent echocardiographgic examination before and after transcatheter closure and during the 3 months and 6 months follow-up. The left ventricular end-diastolic diameter (LVEDD), left ventricular end-diastolic volume (LVEDV), left ventricular end-systolic volume (LVESV), left ventricular stroke volume (LVSV), left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS) were obtained. Results After transcatheter closure, LVEDD, LVEDV significantly increased and LVESV slightly increased. Thus, LVSV, LVEF and LVFS significantly increased, suggesting the presence of an improvement of left ventricular preload and left ventricular function. During follow-up, LVESV was stable while the improvements of left ventricular preload and left ventricular function were maintained. Conclusion Transcatheter closure of ASD is able to effectively improve the left ventricular preload and left ventricular function, and during a short to mid-term follow-up the improvement could be maintained.
