经阴道实时三维子宫输卵管超声造影的效果评价

摘    要:目的 探讨经阴道实时三维超声造影(RT 3D-HyCoSy)评价输卵管通畅性的临床应用价值.方法 对154例不孕症患者共300条输卵管行RT 3D-HyCoSy检查,观察RT 3D-HyCoSy的显影特征,评估输卵管通畅性,其中79例共152条输卵管与腹腔镜下美兰通液结果进行对照.结果 152条输卵管,RT 3D-HyCoSy诊断输卵管阻塞真阳性32例、假阳性6例、真阴性106例、假阴性8例.诊断敏感性80.0%、特异性94.6%,诊断符合率90.8%.一致性检验Kappa值为0.759(P<0.01),一致程度较好.结论 经阴道实时三维子宫输卵管超声造影评价输卵管通畅性安全、简单、可靠,可作为了解不孕症患者输卵管通畅性的主要筛查手段.

Abstract:Objective To investigate the clinical application value of transvaginal real-time three dimensional hysterosalpingo -contrast sonography (RT-3D-HyCoSy) in the evaluating of fallopian tube liqnidity. Methods 154 infertile women (a total of 300 tubes) were underwent transvaginal RT-3D-HyCoSy to observe the image developing characters and assess the liquidity of the fallopian tubes. Among them, the results of 71 patients (a total of 152 tubes) were compared with the results of laparoscopically methylene blue. Results In the 152 tubes, there were 32 true positive cases in the diagnosis of fallopian tube obstruction with RT 3D-HyCoSy, 6 cases of false positive, 106 cases of true negative and 8 cases of false negative. The sensitivity, specificity and accuracy in diagnosing tubal patency by using RT-3D-HyCoSy was 80. 8%, 94. 6%, 90. 8% respectively. The two methods had high consistence and the Kappa value was O. 759. Conclusions Transvaginal RT-3D-HyCoSy was a safe, simple and reliable method in the assessment of fallopian tubes liquidity, which could be used as the primary method for assessing the fallopian tubes liquidity of Infertility patients.
