影像学检查在肠瘘诊治中的指导作用

目的 探讨影像学检查在肠瘘诊治中的指导作用.方法 回顾性分析28例各型肠瘘患者的临床资料,其中经腹壁瘘口造影66次,消化道造影37次,腹部平片31次,腹部CT检查36次,腹部B超检查29次,B超定位穿刺1次,腹部MRI检查4次.结果 28例患者经以上检查均获得明确诊断,19例患者通过反复瘘道造影,指导双腔负压吸引管和水压管摆放位置,瘘口自愈;9例患者经引流后最终行确定性手术治愈.结论 影像学检查对肠瘘患者的诊治有着重要作用.反复多次瘘道造影对瘘道引流通畅及瘘口自愈起着指导作用,消化道造影及CT检查对病人最终确定性手术的时间及手术切口选择能提供可靠依据.

Evaluation of Image Examination in the Diagnosis and Treatment of Intestinal Fistula

Objective To discuss the evaluation of treatment of intestinal fistula. Methods Twenty-eight retrospectively. The image examination included 66 time image examination in the diagnosis and cases of intestinal fistula were reviewed s of fistulogrphy through abdominal wall, 37 times of gastrointestinal radiography,31 times of plain abdominal radiograph, 36 times of ab ominal CT examination, 29 times of ultrasonic-B abdominal examination, 1 time of ultrasound-guided puncturation,4 times of abdominal MRI. Results All 28 patients had final diagnosis by using the image examination. The orificium fistula of 19 patients were cured through adjusting the position of silicone sump drain and weep pipe guided by repeated fistulogrphy, and others were cured by definition operation after effective tance of image examination in diagnosis and draining. Conclusion There could be a great importreatment of the patients with intestinal fistula. Repeated fistulogrphy should be the guidance to making effective draining of fistulous tract and good cure of orificium fistulae. Gastrointestinal radiography and CT examination would be helpful to choosing the time and the incision of definition operation.
