TBIL、TCH、Urea、ALB、PTA等生化指标影响重型肝炎预后分析

目的 :探讨影响重型肝炎预后的因素。方法 :对我院收治的 118例重型肝炎资料进行统计分析。结果 :118例重型肝炎患者死亡 5 7例 (死亡率 4 8 3% ) ;年龄、性别因素不影响重型肝炎预后 (P >0 0 5 ) ;各临床分型间病死率1] 急重肝 >慢重肝 >亚重肝 ,前者明显高于后两者 (P <0 0 1)。有感染、肝性脑病、肝肾综合症、上消化道出血者预后不良 ,且并发症越多预后越差 ;血清总胆红素 (TBIL)越高 ,血清胆固醇 (TCH)、尿素 (Urea)、白蛋白 (ALB)及凝血酶原活动度 (PTA)越低 ,预后越差 (P <0 0 5 )。结论 :在重肝治疗中 ,密切观察重要器官的功能 ,减少并发症的发生 ,反复检测生化指标是改善重症肝炎预后的关键。

The Influence Biochemistry Markers TBIL,TCH,Urea,ALB,PTA on the Prognosis of Serious Hepatitis

Objective: To explore factors affecting serious hepatitis prognosis. Method:The data of 118 inpatients with serious hepatitis were analysed. Results: In 118 cases, fifty seven cases died(48.3%). Age and sex did not affect prognosis of serious hepatitis( P> 0.05).The order of mortality was displayed: acute serious hepatitis > chronic serious hepatitis >subacute serious hepatitis ( P <0.01). Patients complicated with hepatic encephalopathy, hepatorenal syndrome or upper gastrointestinal bleeding had bad prognosis, and the more complications patients got, the worse their prognosis was, the higher TBI, TCH, Urea, ALB , PTA or lower PTA levels were, the worse prognosis they had( P <0.01).Conclusion: In serious hepatitis treatment, it is very important to closely observe function of critical organs for decreasing complications occurrence. Repeated detecting biochemical indicator is key point to improve serious hepatitis prognostic.
