宫颈刮片与阴道镜检查在诊断宫颈上皮内瘤变中的临床研究

目的:探讨宫颈刮片与阴道镜检查在诊断宫颈上皮内瘤变(CIN)的临床应用。方法:通过对386例有接触性出血等临床可疑症状患者分别行宫颈刮片和阴道镜检查,然后以病理组织学诊断为金标准,用敏感性、特异性、假阴性率、假阳性率及诊断符合率进行对比分析。结果:阴道镜检查的敏感性88.17%、特异性89.76%、假阳性率10.24%、假阴性率11.83%、诊断符合率89.38%;宫颈刮片细胞学检查的敏感性49.46%、特异性93.52%、假阳性率6.48%、假阴性率50.54%、诊断符合率82.9%。结论:将宫颈刮片与阴道镜检查联合使用,镜下定位取活体组织进行病理诊断,既避免常规四点活检的盲目性,又能减少各自的假阴性及假阳性,降低漏诊率,提高诊断率。

Clinical Study of Cervical Scraping Smear and Electronic Colposcope Test in Diagnosis of Cervical Intraepithelial Neoplasia

Objective：To investigate the diagnostic value of cervical scraping smear （C.SS） and electronic colposcope （EC） in patients with cervical intraepithelial neoplasia （CIN）. Methods： A total of 386 female patients with contact bleeding were examined with CSS and EC, regarded pathohistology as gold standard, compared with sensitivity, speci- ficity , false negative rate, false positive rate and diagnose accordance rate. Results： The sensitivity, specificity, false negative rate,false positive rate and diagnose accordance rate of EC were 88. 17% ,89. 76%, 10. 24%, 11.83%, 89.38%. The sensitivity,specificity , false negative rate, false positive rate and diagnose accordance rate of CSS were49. 46%, 93. 52%, 6. 48%, 50. 54% and 82. 9%. Condusion：CSS in combination of EC may cut down the rate of missed diagnosis and increase the early diagnosis rate.
