老年糖尿病患者中甲状腺疾病的现患调查

目的:探讨老年糖尿病患者中甲状腺疾病的现患情况。方法:横断面调查219例老年糖尿病患者和215例非老年糖尿病患者的甲状腺功能,部分患者检查甲状腺B型超声。结果:(1)老年糖尿病患者中,甲状腺疾病患病率为27.40%,性别差异不显著,其中临床甲亢、甲减、亚临床甲亢患病率分别为2.28%、5.48%、4.11%,亚临床甲减患病率最高(15.53%);(2)老年糖尿病合并甲状腺疾病患者中,亚甲亢、亚甲减患病率明显高于非老年糖尿病组患者(P<0.05),甲亢低于非老年糖尿病组患者(P<0.05),两组甲减的差异不显著;(3)老年糖尿病患者中甲状腺结节患病率为51.79%,性别差异不显著,患病率明显高于非老年糖尿病组患者(P<0.05)。结论:老年糖尿病患者合并甲状腺疾病较常见,并可能对糖尿病的预后及治疗有一定影响,筛查和随访糖尿病患者的甲状腺功能,并探讨糖尿病合并甲状腺疾病的发病机理具有重要的临床意义。

The Prevalence Study of Thyroid Disease in Geriatric Diabetes Patients in Jiangsu

Objective:To describe the prevalence rate of thyroid disease in geriatric patients with diabetes.Methods:A cross-sectional study was underwent among 219 geriatric diabetic patients and 215 no-geriatric patients,serum thyroid function was measured and thyroid ultrasound was estimated in some of them.Results:(1)The prevalence rate of thyroid disease was 27.40% in 219 geriatric diabetic patients,no significant differences was found between male and female.The prevalence rate of subclinical hypothyroidism was 15.53 %,which was much higher than that of other thyroid diseases(clinical hyperthyroidism 2.28%,clinical hypothyroidism 5.48% and subclinical hyperthyroidism 4.11%,respectively).(2)The prevalence rates of subclinical hyperthyroidism and subclinical hypothyroidism in geriatric diabetic patients were significantly higher than those of no-geriatric diabetic patients(P<0.05),while the rate of hyperthyroidism was lower than that of no-geriatric diabetic patients(P<0.05),and the prevalance of hypothroidism was not found significantly difference between geriatric and no-geriatric diabetic patients.(3)The prevalence rate of thyroid nodules in geriatric diabetic patients was 51.79%,which was significantly higher than that of no-geriatric diabetic patients(P<0.05),but no significant difference was found between male and female.Conclusion:It is common that thyroid disease is combined with diabetes in geriatric diabetic patients,which may affects the results and treatment.It is important to check out and follw up thyroid function and thyroid ultrasound in geriatric diabetic patients.Meanwhile to explore pathogenesis of diabetes mellitus combined with thyroid disease has important clinical value.
