银杏达莫联合ACEI治疗糖尿病肾病的Meta分析

目的:对银杏达莫联合血管紧张素转化酶抑制剂(ACEI)治疗糖尿病肾病疗效进行Meta分析。方法:检索国内公开发表的有银杏达莫与ACEI联合治疗糖尿病肾病的随机对照试验(RCT),采用Cochrane系统评价手册进行质量评价,用RevMan 5.1.4软件进行Meta分析。结果:共纳入12项研究分析,银杏达莫与ACEI联合用药治疗组在降低尿白蛋白排泄率(UAER)、血尿素氮(BUN)、血肌酐(SCr)、血浆黏度以及血纤维蛋白原等方面优于单用ACEI组(P<0.05),但在降低血甘油三酯(TG)、胆固醇(TC)方面无统计学意义(P>0.05)。结论:银杏达莫与ACEI联合用药对早期糖尿病肾病有一定疗效,但由于现有研究质量存在一定方法学问题,尚需要严格设计的大样本RCT加以证实。

Meta-analysis of Ginkgo Injection Combined with ACEI in the Treatment of Diabetic Nephropathy

Objective:To evaluate the effectiveness of ginkgo injection combined with angiotensin-converting enzyme inhibitors(ACEI) for treating early diabetic nephropathy(DN).Methods:Randomized controlled trials(RCT)on ginkgo injection combined with ACEI for treating early DN were included by the standard from Cochrane Reviewers’ Handbook 4.2.2 and analyzed by Rev-Man 5.1.4.Results:12 RCTs were included in Meta analysis,compared with control group,the combining treated group was better than ACEI group in reducing urinary albumin excretion rates(UAER),Blood urea nitrogen(BUN),Serum creatinine(SCr),Fasting plasma glucose(FPG),Plasma specific viscosity and fibrinogen(P<0.05),but there was no significant difference in controlling triglyceride(TG) and total cholesterol(TC)(P>0.05).Conclusion:Meta-analysis manifests that ginkgo injection has certain curative effect on early DN.Analytic results should be verified by strictly-designed and large-scale sample randomized control trial because some methodological problems were existed in present research.
