声门上型喉癌对声门旁间隙的侵犯

目的:探讨声门上型喉癌对声门旁间隙侵犯的组织病理学特点,为声门上型喉癌手术提供理论依据。方法:66例声门上型喉癌喉切除标本,经火棉胶包埋,制成连续切片,苏木精-伊红染色,光学显微镜下观察。结果:声带活动良好、声带活动受限及声带固定病例中对声门旁间隙下区的侵犯率分别为0(0/21),26.0%(7/27),55.6%(10/18),三者差异有统计学意义(P<0.05);淋巴结转移组声门上型喉癌对声门旁间隙下区的侵犯率高于无淋巴结转移组喉癌(P<0.05);喉全切除术病例中侵犯声门旁间隙上、下区的侵犯率均高于喉部分切除术病例(P<0.05)。结论:声门上型喉癌对声门旁间隙下区侵犯与淋巴结转移及声带活动情况有关,并影响到手术术式的选择。

Paraglottic space invasion of supraglottic carcinoma

Objective:To explore the histopathological characteristics of supraglottic carcinoma with paraglottic space invasion,and to provide the theoretical basis for the operation of supraglottic laryngeal carcinoma. Methods:Sixty-six specimens from patients with supraglottic laryngeal carcinoma were imbedded in collodion,made whole-organ serial section for HE staining,and observed under light microscope. Results:The rates of infraglottic space invasion were 0 in 21 cases of vocal cord with good movement,26.0% in 27 cases of vocal cord limitation,and 55.6% in 18 cases of vocal cord fixation,and the difference was significant between these 3 groups (P < 0.05). The rate of infraglottic space invasion in supraglottic carcinoma with lymph node metastasis was higher than that in supraglottic carcinoma without lymph node metastasis (P < 0.05). The rate of paraglottic space invasion of total laryngectomy was higher than that of partial laryngectomy (P < 0.05). Conclusion:The infraglottic space invasion of supraglottic carcinoma is closely correlated with vocal cord movement and lymph node metastasis,and it may affect the choice of operation method.
