血清乳酸脱氢酶检测对溶血性贫血诊断意义的临床研究

目的：探讨溶血性贫血(溶贫)患者血清乳酸脱氢酶(LDH)的相关临床意义。方法：采用酶法随机对41例初诊溶贫患者血清LDH活性进行检测并与非溶血性疾病所致的贫血(非溶贫)患者血清LDH活性进行对比。结果：41例溶贫患者中36例血清LDH活性高于正常上限，阳性率为87．80％；非溶贫组72例患者中8例血清LDH活性高于正常上限，阳性率为11．11％。溶贫组血清LDH阳性率明显高于对照组(P＜0．001)。溶贫组36例LDH阳性者中5例伴肝功能损害。溶贫组轻度、中度、重度贫血患者LDH活性有明显差异(P＜0．05)。有效治疗后，LDH活性比治疗前显著下降(P＜0．001)。结论：血清LDH活性增高可作为溶贫筛选诊断的参考指标之一。溶贫患者血清LDH活性增高系因红细胞溶解所致并与贫血程度成正比关系。溶贫患者血清LDH活性的改变可用于评判药物疗效、指导治疗、估计病情。

Study on the clinical significance of assaying activity of serum lactic dehydrogenase in patients with hemolytic anemia

Objective: To explore the clinical significance of elevated serum lactic dehydrogenase (LDH) activity in patients with hemolytic anemia (HA) Methods: The enzymic method was used to assay serum LDH activity in 41 cases of patients with HA randomly, which was compared with that of the patients with anemia caused by non hemolytic disease (non HA) Results: Among 41 patients with HA LDH activity in the serum of 36 exceeded the normal up limit,at a positive rate of 87 80%,while the activity in serum in 8 cases of 72 patients with non HA exceeded the normal range, at a positive rate of 11 11% The positive rate of LDH activity in HA group was obviously higher than that in the non HA group (P<0 001) After effective treatment, LDH activity distinctly decreased (P<0 001) There was significant difference in the LDH activity of patients with respectively slight, moderate and serious anemia in the HA group (P<0 05) Conclusion: LDH activity may be used as index for the diagnosis of HA The increase of LDH activity of patients with HA is related to hemolysis and the degree of anemia The test of LDH activity may be used to analyze HA and assess the effect of treatment and prognosis of HA
