凉血消痤饮治疗寻常痤疮疗效观察及作用机制初探

目的观察凉血消痤饮治疗寻常性痤疮的临床疗效及其对性激素水平和免疫指标的影响。方法寻常痤疮患者115例,随机分为治疗组（57例）和对照组（58例）,治疗组内服凉血消痤饮,对照组口服西药米诺环素,另选50例同年龄无痤疮人群为正常对照组,观察血清卵泡刺激素（FSH）、黄体生成素（LH）、睾酮（T）、雌二醇（E2）、IgA、IgG、IgM、C3、C4水平在治疗前后的改变。结果治疗组和对照组总有效率分别为94.8%和74.2%,两组比较有显著性差异。男性痤疮患者治疗前LH和FSH水平显著低于正常组（P〈0.001）,治疗后其LH及FSH水平与正常对照组无显著性差异（P〉0.05）。女性痤疮患者治疗前LH和FSH水平低于正常组而T水平高于正常组,差异有显著性（P〈0.001）,治疗后FSH及T水平与正常组无显著性差异（P〉0.05）。免疫指标IgG治疗前后有显著性差异。结论凉血消痤饮治疗寻常痤疮疗效确切,可能通过调节机体性激素水平和免疫功能等多种途径达到治疗寻常痤疮的目的。

The study on the therapeutic effects and the correlated mechanism of using Liangxuexiaocuoyin to treat the patients with acne vulgaris

Objective： To observe the clinical therapeutic effect of Liangxuexiaocuoyin in treating acne vulgaris, to evaluate objectively the sexual hormone and IgA, IgG, IgM, C3 , C4 in patients with acne vulgaris and to assess the effect of Liangxuexiaocuoyin on sexual hormone and immune system. Methods： One hundred and fifteen patients were randomly divided into the treated group 0（ n = 57 ） and the control group （n = 58 ）, which were treated with Liangxuexiaocuoyin and Western medicine （minocycline） respectively. Besides,50 healthy subjects at the same age were selected as the normal control. Serum levels of follicle stimulating hormone （ FSH ）, luteinizing hormone （LH） , testosterone （ T）, estradiol （ E2 ）, IgA, IgG, IgM, C3 and CA in all the patients and healthy subjects as well as the clinical therapeutic effect of the treatments were observed and compared. Results： The total effective rates in the treated group and the control group were 94.8% and 74.2% respectively. Comparison between them showed significant difference. Serum levels of FSH, and LH in the male patients before treatment were lower than those in the healthy subjects （P 〈 0. 01 ） , while showed no difference between the treated group and the healthy subjects after treatment（ P 〉 0. 05 ）. In the female patients, serum levels of FSH and LH before treatment were lower than those in the healthy subjects （P 〈0. 01 ） ,but the T serum level was higher than that in the healthy subjects （P 〈0. 01 ）, While showed no difference between the treatment group and the healthy subjects after treatment（ P 〉0. 05 ）. There was significant difference in the serum level of IgG before and after the treatment with Liangxuexiaoeuoyin （P 〈 0.01 ）. Conclusion： Liangxuexiaocuoyin has definite clinical effect in treating acne vulgaris. The therapeutic mechanism of Liangxuexiaocuoyin may be credited with changing the sexual hormone and the immune system.
