局部晚期肺癌的手术适应症及手术方式探讨

目的:探讨局部晚期肺癌的手术适应症及手术方式。方法:回顾分析了我院1991年1月~2005年6月对45例局部晚期肺癌的手术治疗情况,并对手术适应症及手术方式进行了探讨。结果:合并脑转移的8例周围型肺癌先行脑转移瘤切除,后行肺叶切除加淋巴结清扫术。术后并发包裹性脓胸1例,气胸2例,支气管肺炎2例。急性呼吸衰竭死亡1例。经随访1 a,3 a,5 a生存率分别为53.66%(22/41),30.30%(10/33),14.29%(4/28)。结论:对适当的局部晚期肺癌患者,选择合适的手术治疗可能使患者获得较长期的生存机会。

On the operation indication and operation way for local advanced lung cancer

Objective: To probe into the operation indication and operation way for local advanced lung cancer.Methods: The surgical treatment results for 45 cases of local advanced lung cancer in our hospital from Jun.1991 to June 2005 were analyzed retrospectively.,and the operation indication and operation way for this sorts of patient were discussed.Results: For the 8 cases of peripheral type of lung cancer whom had have brain metastasis we did metastatic tumor resection first in the neurosurgery department.Then lobectomy of lung and lymph nodes cleaning up were made in our department.The postoperative complications were including encapsulated empyema in 1 case,pneumatothorax in 2 cases,pneumonia in 2 cases,and one case died due to acute respiratory failure.The follow-up results showed the 1,3and 5 years survival rate were 53.66%(22/41),30.30%(10/33)and 14.29%(4/28)separately.Conclusion: For the adequate local advanced lung cancer patient,suitable operation way for treatment may gain a opportunity for lounger survival period.
