基于心血管疾病的远程医疗临床应用与分析

目的：通过3215例心血管远程会诊病例分析，探讨以远程会诊、远程教育培训等信息化方式帮扶新疆区域基层医疗卫生机构，提高新疆地区整体心血管防诊治水平。方法：对新疆医科大学第一附属医院自2008年4月至2012年12月的心血管远程会诊病例进行分析，统计年、月会诊人数，会诊分类特点及会诊疾病谱和会诊疾病地方差异。结果：会诊量呈逐年增加趋势；会诊以普通会诊多见，占会诊总量的78．2％；心血管会诊疾病较多的是冠心病、心律失常、瓣膜性心脏病和高血压病；各地区会诊量分布不均，伊犁地区、阿克苏地区、阿勒泰地区、喀什地区、巴音郭楞蒙古自治州会诊量较大，占总会诊量的79．8％。结论：随着医院远程医疗中心的建立和会诊制度的完善，心血管疾病会诊量呈逐年上升趋势，但仍有地区会诊量较少，是今后工作的重点，会诊疾病仍以多发病常见病为主，提示要加强各地区心血管疾病的基础知识教育并进行临床经验交流，提高当地心血管疾病的诊治能力。

Clinical Application and Analysis of Telemedicine Based on Cardiovascular Disease

Objective： By analyzing 3,215 cases of cardiovascular remote consultation, helping basic-level medical and health organization in Xinjiang Region with remote consultation, remote education training and other informationization methods to improve the overall level of cardiovascular prevention, diagnosis and treatment of Xinjiang Region is discussed about. Method： Cardiovascular remote consultation cases of the First Teaching Hospital of Xinjiang Medical University from April 2008 to December 2012 were analyzed. The number of patients consulted each year and month was calculated. Consultation classification and characteristics, consulted disease spectrum and local differences of consulted disease were discussed about. Results： The consultation quantity has trend of increase year by year; the consultation is mainly ordinary consultation, accounting for 78.2% of the total consultation; cardiovascular diseases consulted mainly include coronary heart disease, arrhythmia, valvular heart disease and hypertension; and regional consultation quantity has uneven distribution. Ili Region, Aksu Region, Altay Region, Kashgar Region and Bayingolin Mongol Autonomous Prefecture have a large consultation quantity, accounting for 79.8% of the total consultation quantity. Conclusion： With the establishment of telemedicine center and the improvement of consultation system of our hospital, there is an upward trend in consultation quantity of cardiovascular disease, but some regions have a small consultation quantity, which are the key of the future work. The consulted diseases are still mainly with frequently occurring diseases and common diseases. It is suggested to strengthen basic knowledge education of cardiovascular diseases in different regions, exchange clinical experiences, and improve the ability of local diagnosis and treatment of cardiovascular diseases.
