单病种引导信息化支撑的区域性医疗资源纵向整合探索

为发挥区域内各级医院有效资源，使病员得到优质、便捷的医疗服务，进一步利用已经建设的区域医疗信息共享系统，开发了以单病种为纽带，信息系统为支撑的肿瘤诊疗业务临床协同系统。系统以规范的单病种诊疗指南为基础，整合区域内医疗信息数据，建立及时优质的分级医疗和规范、连贯性的转诊医疗服务流程及动态医疗信息共享。在不改变当前体制机制的情况下，探索出在区域居民健康档案网络平台上，充分利用信息资源，开展慢病按病种诊疗协同服务的新模式，为医改要求的实施区域性医疗资源整合提供新途径。

The Exploration of Regional Medical Rresources Vertically Integration Supported by Single Disease Informationization

In order to use the resources of hospitals at all levels effectively and make patients get high quality and convenient service, we developed clinical collaboration system for cancer treatment, which is linked by single disease and supported by information system. This system could build a high quality medical model, standardized and coherent referral service process, and dynamic sharing medical information. It integrates regional medical information data based on standard treatment guidelines of single disease. This project explores a new model of treatment of chronic diseases by res collaborative services on the network platform of patients＇ health ources integration that is required by medical reform without chang files. It provides a new way for regional medica ng the current mechanism.
