支气管、肺动脉、气管隆凸成形术治疗中央型肺癌

目的：探讨中央型肺癌行支气管、肺动脉、气管隆凸成形术的可行性和效果。方法：对116例中央型肺癌患者行支气管、肺动脉、气管隆凸成形术，其中对61例采用支气管或肺动脉成形术，5例采用隆凸、支气管成形术，50例采用支气管肺动脉双袖状成形术。结果：术后患者的1、3、5、10年生存率分别为90．7％（98／108）、61．2％（49／80）、41．2％（14／34）、25％（2／8）。发生手术并发症19例（19／116，16．4％），死亡2例（2／116，1．7％）。结论：对中央型肺癌选择肺叶或全肺切除同时进行支气管、肺动脉、气管隆凸成形术能有效清除病灶，并能最大限度地保留健康肺组织，有利于延长患者的生存期。

Bronchoplasty tracheo-carinal and vascular reconstruction in the treatment of lung cancer

Objective: To explore the feasibility and result of operation such as tracheo-carinal reconstruction,bronchoplasty and vascular reconstruction in selected patients with centrally located lung cancer.Methods: Tracheo-carinal reconstuuction and bronchoplasty were performed in 5 cases,pulmonary arterioplasty and double sleeve resection were performed in 61 cases and 50 cases respectively,from Mar,1994 to Mar,2004.Results: Postoperative complications occurred in 19 patients(16.4%) and operative death ouccurred in 2 cases(1.7%).The 1,3,5 and 10 year survival rate were 90.7%(98/108),61.2%(49/80);41.2%(14/34) and 25%(2/8),respectively.Conclusion: Bronchoplasty,pulmonary arterioplasty,double sleeve resection and tracheo-carinal reconstuction are feasible for selected patients with centrally located lung cancer.It is helpful to prolong the long-term survival of patients with centrally located lung cancer.
