70岁以上脑出血39例临床分析

目的:探讨70岁以上脑出血的病因、临床特点及预后。方法:对2003~2006年在我科及急救中心收治的70岁以上第一次发病的39例脑出血患者的临床资料进行回顾性分析。结果:70岁以上脑出血中非高血压性病因者11例(28.20%),安静状态下发病者10例(25.64%),头痛、呕吐8例(20.51%),意识障碍11例(28.20%),平均出血量44.2 ml,破入脑室14例(35.90%),死亡21例(53.84%)。死亡原因以多脏器功能衰竭最为常见,占76.20%(16例)。结论:70岁以上脑出血患者的临床表现不典型,应尽早行相关检查确诊,防治多脏器功能衰竭,以期降低其死亡率。

Clinical analysis of intracerebral hemorrhage in 39 patients over the age of 70

Objective: To study the clinical features of intracerebral hemorrhage(ICH) in patients over the age of 70.Methods: A retrospective case analysis was done on clinical features of 39 patients with ICH aged over 70,hospitalized in neurological department and emergency center during the year of 2003 to 2006;literatures concerned with the causes,clinical features and prognosis were also reviewed.Results: ICH often occurs at the silent state from nonhypertensive cause,and the occurrence of headache,vomiting and loss of consciousness were not very common in this age group.The amount of hemorrhagic blood was massive and prone to go ventricles easily.Mortality rate is high from multiorgan failure.Conclusion: Clinical manifestations of ICH in patients older than 70 were not very typical,sometime mimicking ischemic stroke and necessitating earlier and prompt diagnosis.Prevention of multiorgan failure is vital to decrease mortality.
