摘    要:目的: 研究一种简便、准确、易于临床推广的儿茶酚胺(CAs)检测方法,进行尿中 CAs测定,探讨 2 型糖尿病肾病(DN)动态血压(ABP)与尿儿茶酚胺(UCA)的关系。方法: 利用高效液相色谱及荧光检测器,对正常人和糖尿病肾病病人24 h尿中去甲肾上腺素(NE)、肾上腺素(E)和多巴胺(DA)进行测定。结果: NE浓度在3~15 ng/ml范围及E浓度在0.3~1.5 ng/ml、DA浓度在1.5~120 ng/ ml范围内时,与色谱峰峰高的线性关系良好( r=0.999 1);NE、E回收率为 75%~82%,DA回收率为 76%;NE、E、DA的精密度 RSD(%)分别为 1.11%、7.21%、1.43% (n=5)。结论: 高效液相色谱法检测尿CAs具有简单、准确、特异性高的优点。通过对正常人及糖尿病肾病病人24 h尿样分析,不能认为交感神经肾上腺髓质系统活性与糖尿病肾病高血压及血压昼夜节律异常之间有相关性。

Abstract:Objective: By using a new method of determination of catecholamines in urine, we studied the determination method of catecholamines in urine by high performance liquid chromatography-fluorescence detection. In order to disscuse the relationship of urinary catecholamines excretion to ambulatory blood pressure in type 2 diabetic nephropathy patients. Methods: Noradrenaline (NE), adrenaline (E) and dopamine (DA) were determined to healthy subjects and cases of type 2 diabetic nephropathy patients in 24h urine by high performance liquid chromatography-fluorescence detection in the best condition. Results: Linear calibration curves for NE, E, DA were measured within a range of 3~15 ng/ ml, 0.3~1.5 ng/ ml (NE, E), 1.5~120 ng/ ml (DA). The correlation coefficient was (r=0.9991). The recovery rates of NE, E and DA were between 75%~82% (NE, E) and 76% (DA). The RSD (%) was 1.11%, 7.21%, 1.43% (N=5). Conclusions: The method was simple, accurate, high specificity. According to the results of NE, E, DA inhealthy subjects and cases of type 2diabetic nephropayhy patients, sympathoadrenomedullary systems seem to have little significant contribution to the pathogenesis of diabetic hypertension and dysregulation of circadian rhythm of blood pressure. It suggests that this method is a useful tool for diagnosis of pheochromocytoma.
