原发性气管、隆突部肿瘤的诊断及体外循环在术中的应用体会

目的：总结气管、隆突肿瘤外科治疗经验,探讨其手术相关技术的合理应用。方法：回顾性分析2001年2月～2010年6月经手术治疗的46例气管、隆突肿瘤患者的临床资料,研究气管、隆突肿瘤外科治疗的疗效,总结其诊断及体外循环在其手术中应用的临床经验。结果：气管环形切除＋对端吻合33例,气管＋隆突切除重建12例,气管楔形切除1例;无围术期死亡,临床治愈39例（84.8%）;术后随访7个月～7年,死亡5例（10.9%）,生存至今40例（86.9%）,其中生存5年及以上者9例（20.0%）,生存3年以上者21例（46.7%）,2例带瘤生存已达1年。结论：气管、隆突肿瘤外科手术疗效确切,早期诊断很关键,合理应用体外循环技术可提高手术安全性。

Clinical experience of diagnosis of primary tumors of the trachea and carina and the application of cardiopulmonary bypass in the operation

Objective：To accumulate experience of surgical treatment of tracheal or carinal tumors and reasonable application of assisted technology during the operation.Methods：Retrospective review of 46 patients with trachea tumor or carina underwent surgical treatment from February 2001 to June 2010,researched the outcome of surgical intervence,summarized the clinical experience of the diagnosis and application of cardiopulmonary bypass.Results：Resection and reconstruction of trachea in 33 patients,carinal resection and reconstruction in 12 patients,and trachea wedge resection in 1 patient.No patient died perioperatively.39 cases（84.8%） were clinical cured.Follow-up 7 months to 7 year,5 cases（10.9%） were died,40 cases（86.9%） survived,with 9 cases（20.0%） over 5 years,21 cases（46.7%） over 3 years,2 cases were still alive with tumor over 1 year.Conclusion：The efficacy of surgical treatment of tracheal or carinal tumors is exact,early diagnosis is very important,the operation can be more safe by reasonable assist of cardiopulmanory bypass.
