摘    要:目的 探讨膜引导技术在即刻种植牙中的临床疗效,以及对患者骨修复的影响.方法 选择浙江省台州市温岭第三人民医院即刻种植牙患者38例(种植牙体52枚)作为研究对象,观察患者的临床疗效以及对不同方法测量计算成骨率进行比较.结果 52颗牙钛膜平均保留时间为3.2个月,种植体稳定,所有切口均为甲级愈合,随访两年,患者种植体周围骨的吸收高度平均为(1.31±0.17)mm,未出现松动情况;平均骨厚度测量结果:直接测量法为(2.20±0.27) mm,CT二维测量法为(2.27±0.30)mm,两种方法经相关性分析呈显著正相关(r=0.9994,P< 0.01);直接测量方法计算成骨率为(89.01±1.26)%,采用CT二维测量法计算成骨率为(88.82±1.97)%,采用CT三维容积法计算成骨率为(86.37±2.15)%,CT三维容积法显著低于直接测量法和CT二维测量法(P<0.05),而直接测量法与二维测量法之间差异无统计学意义(P>0.05).结论 膜引导技术运用于即刻种植中,能够获得良好的骨修复,CT三维重建能够对种植效果进行客观准确的评价,同时还能够为术前的方案设计提供参考,值得在牙种植中推广使用.

Abstract:Objective To investigate the clinical effects of membrane guided technology in the immediate implant and the impact on bone repair.Methods 38 patients who had accepted immediate dental implant (52 implant bodies) from the Third People's Hospital of Wenling City were selected as study object,whose clinical effects were observed and the bone formation rates would be calculated by different methods and these methods were compared.Results All the teeth titanium membrane was kept in good state during an mean time of 3.2 months,the implants were stable,all incisions healed well to the Class A healing rate.During the following two years,it was also found that the absorption altitude of the bone s around the implants was (1.31±0.17) mm in average without loosing.The mean bone thickness was (2.20±0.27) mm by direct measurement and (2.27±0.30) mm by CT two-dimensional measurement,both of which showed a significant positive correlation (r =0.9994,P <0.01).The bone formation rates was (89.01±1.26)% by direct measurement,(88.82±1.97)% by CT two-dimensional measurement,and (86.37±2.15)% by CT three-dimensional volumetric method.The statistics calculated by CT three-dimensional volumetric method were significantly lower than those by direct measurement method and CT two-dimensional measurement (P < 0.05),while there were no statistically significant differences between the other two methods (P > 0.05).Conclusion Membrane guided technique used in immediate dental implant is good for the bone repair,and three dimensional CT reconstruction can reveal the average effects in an objective and accurate way,and offer reference in the project design before an operation,thus it is worthy of recommending and popularizing in dental implant.
