120例自控硬膜外镇痛分娩临床观察

目的：探讨自控硬膜外镇痛（PCEA）对产妇及新生儿的影响。方法：对2008年9月～2009年9月我院120例行硬膜外分娩镇痛、健康、单胎头位、足月临产初产妇的产程、疼痛程度、分娩方式、产后出血、新生儿窒息等资料进行回顾性分析。结果：镇痛组活跃期较对照组缩短,剖宫产率较对照组低;镇痛组疼痛程度比对照组明显减轻,两组比较有显著性差异（P〈0.05）。两组催产素使用率、阴道助产率、产后出血量、新生儿窒息率比较无显著性差异（P〉0.05）。结论：自控硬膜外分娩镇痛对分娩过程及分娩结局均无不良影响,既能有效地缩短产程,又能有效地降低剖宫产率,是目前效果较确切和比较安全的方法,值得临床推广应用。

Clinical observation of controlled epidural analgesia in labor of 120 cases

Objective：To discuss the impact of patient-controlled epidural analgesia on maternal and newborns. Methods：Retrospective analysis was conducted in 120 cases of epidural analgesia in labor,health,single fetal head position,the term of the provisional maternal the time of labor course,the pain of childbirth,mode of delivery,the amount of postpartum hemorrhage,asphyxia neonatal from September 2008 to September 2009 in our hospital. Results：Active time of analgesic group than that of the control group shortened,the rate of cesarean section decreased than that of the control group. Analgesia group level and anxiety alleviated than that of the control group,there were significant differences between them（P〈0.05） . The rates of oxytocin usage,vaginaldelivery,the amount of postpartum hemorrhage and the asphyxia neonatal in the two groups showed no significant difference（P〉0.05） . Conclusion：No adverse effects of labor analgesia is found on delivery process and outcome about the controlled epidural labor analgesia. It could shorten labor stage and decrease the cesarean section rate effectively. It is a kind of relatively effective and safe method,which is worthy of clinical use.
