急性颅脑损伤CT评分与血糖、乳酸脱氢酶和超敏C反应蛋白表达关系的研究

摘    要:目的观察急性颅脑损伤患者的CT评分与血糖、乳酸脱氢酶和超敏C反应蛋白(hs-CRP)的关系,以期为临床工作提供理论支持。方法选择88例急性颅脑损伤的患者作为观察组,进行螺旋CT评分,选取50例正常成人作为对照组,检测两组患者血清中血糖、乳酸脱氢酶和hs-CRP的表达,探讨其与病变程度的关系。结果急性颅脑损伤患者有明显的CT特征性表现,且血清中血糖(11.35±2.21)μmol/L vs(5.14±1.32)μmol/L]、乳酸脱氢酶(315.47±28.64)U/L vs(212.09±22.87 U/L)]和hs-CRP(5.53±1.02)mg/L vs(1.02±0.26)mg/L]的表达水平较对照组升高(P〈0.05);观察组中血糖、乳酸脱氢酶和hs-CRP的表达与GCS分期、中线情况及环池情况密切相关(P〈0.05);线性相关分析显示,CT评分均与血糖、乳酸脱氢酶和hs-CRP呈正相关(P〈0.05)。结论急性颅脑损伤时CT诊断有特异性,且血清中血糖、乳酸脱氢酶和hs-CRP的表达升高,CT联合血糖、乳酸脱氢酶和hs-CRP的检测对早期判断病变程度及预后有重要价值。

Abstract:Objective To investigate the correlation between CT scores and blood glucose,lactate dehydrogenase and C-reactive protein(hs-CRP) in patients with acute traumatic brain injury,in order to provide theoretical support for clinical practice.Methods 88 cases of patients with acute traumatic brain injury were chosen as the observation group and were scored by spiral CT.50 cases of normal adults were chosen as the control group.Levels of serum glucose,lactate dehydrogenase and hs-CRP expression of both groups were detected and their relationships with the severity of disease were explored.Results Compared with the control group,acute traumatic brain injury patients had clear CT features and had elevated level of serum glucose (11.35±2.21) μmol/L vs(5.14±1.32) μmol/L],lactate dehydrogenase (315.47 ± 28.64) U/L vs(212.09±22.87) U/L] and expression of hs-CRP (5.53±1.02) mg/L vs(1.02±0.26) mg/L](P < 0.05).Serum glucose,lactate dehydrogenase and expression of hs-CRP in the observation group were closely related to GCS stage,midline situation and ambient cistern(P < 0.05).Linear correlation analysis showed that CT score was positively correlated with glucose,lactate dehydrogenase and expression of hs-CRP(P < 0.05).Conclusion Acute brain injury has specific CT features and has elevated level of serum glucose,lactate dehydrogenase and hs-CRP expression.CT combined with detection of blood glucose,lactate dehydrogenase and hs-CRP expression have significant value for the early determination of disease severity and prognosis.
