交锁髓内针固定股骨干骨折的疗效及生物力学原理研究

摘    要:目的:观察交锁髓内针固定股骨干骨折的临床疗效。方法:回顾性分析我院2008年3月~2010年4月收治的交锁髓内针固定股骨干骨折患者88例临床资料,随机选择同期行AO钢板内固定治疗,比较两组疗效及分析内固定物生物力学效应。结果:交锁髓内针固定患者在手术时间、住院时间、骨愈合时间、失血量等方面与AO钢板内固定组比较差异明显(P〈0.05、P〈0.01);交锁髓内针固定具有动性和静力性的加压作用,最大限度地减少应力遮挡效应,其固定作用更加牢固。结论:交锁髓内针固定具有手术创伤小、操作简单和固定牢固的优点。它除了具备有骨折端加压作用及较强的抗负载作用外,有很好的抗旋转作用,使股骨颈骨折固定更加牢固,提高了骨折愈合率。

Abstract:Objective: To observe the efficacy of interlocking intramedullary nail in the treatment of femoral shaft fracture.Methods: 88 patients who were locked by intramedullary nail in the treatment of femoral shaft fracture in our hospital from March 2008 to April 2010 were retrospectively analyzed,and patients with AO steel plate internal fixation at the same time were randomly selected.the Efficacy and biomechanical effect of the two groups were compared Results: The operative time,hospitalization time,bone healing time,blood loss,and other aspects of the two groups had significantly different(P〈0.05,P〈0.01);interlocking intramedullary nail had advantage in dynamic and static pressure of the role,to minimize the stress shielding effect,and it was more firmly fixed.Conclusion: Interlocking intramedullary nail has advantage in simple operation and stable internal fixation.It plays a role in a compression fracture and the strong anti-load effects,have a good anti-rotation effect in that the femoral neck fracture and is fixed more firmly to improve the healing rate.
