老年髋关节骨折术后死亡率的分析研究

目的：探讨老年髋关节骨折手术治疗的术后死亡率及死亡原因。方法：选取2009-03~2012-12期间我院关节外科手术治疗髋关节骨折的214例老年病人（65~95岁）开展术后随访,对其临床病历资料进行回顾性分析研究。分析总体死亡率及术后1a内死亡率,总结其构成特点及死亡原因。结果：髋关节手术治疗术后的总体死亡率为10.28%（22/214）,术后1a内死亡率为7.48%（16/214）。术后1a内死亡人数占总体死亡人数的43.75%。影响术后死亡率的主要因素为性别（字2=3.546,P=0.044）、年龄（字2=12.417,P=0.027）、呼吸功能衰竭及心脑血管疾病意外。此外, ASA 评分高的病人术后死亡率较低的病人增高,差异有统计学意义（ P〈0.05）。结论：手术治疗老年髋关节骨折的术后死亡率较低。同时积极地配合康复锻炼,有助于降低术后死亡率。

THE ANALYTICAL INVESTIGATION OF THE MORTALITY IN THE SENILE PATIENT WITH HIP FRACTURES TREATED BY OPERATIONS

Objective：To discuss the mortality in the senile patients with hip fractures who have been treated by operations. Methods：We collected the clinical data of 214 senile patients （ 65 to 95 years old） with hip fracture. Who had been treated by operations from March 2009 to September 2012 in the articular surgery of our hospital. Their overall mortality and one-year mortality were analyzed to find the causes for death. Results：Twenty-two cases died and the overall mortality was 10. 28%（22/214）. Sixteen patients died within one year and the one-year mortality was 7 . 48% （ 16/214 ） . The postoperative deaths within one year accounted for 43. 75% of overall deaths. The main factors influencing the postoperative mortality for gender （X2 =3 . 546 , P=0 . 044 ） , age （X2 =12 . 417 , P=0. 027）,respiratory function failure and disease of heart head blood-vessel accident. Additionally,there was significant difference between ASA scores and postoperative mortality（P〈0. 05）. Conclusions：Op-erations can lead to a low mortality for senile patients with hip fractures. Actively cooperate with reha-bilitation exercise,helps to reduce the postoperative mortality.
