瑞芬太尼与丙泊酚复合液在门诊无痛胃镜中的应用

目的观察瑞芬太尼复合丙泊酚在门诊无痛胃镜中的应用效果。方法30例门诊无痛胃镜检查患者随机分为对照组和观察组各15例。对照组静脉注射丙泊酚2 mg.kg-1进行诱导,手控速度2 mg.s-1,待患者神志消失后用微量泵静脉注射丙泊酚2 mg.kg-1.h-1,必要时缓慢追注丙泊酚20-30 mg;观察组静脉注射丙泊酚2 mg.kg-1进行诱导,手控速度2 mg.s-1,待患者神志消失后缓慢静注瑞芬太尼0.1μg.kg-1之后微量泵静脉注射丙泊酚2 mg.kg-1.h-1+瑞芬太尼4.0μg.kg-1.h-1。观察麻醉效果、患者定向力恢复时间以及麻醉对患者血氧饱和度和脉搏的影响。结果观察组麻醉效果满意率明显高于对照组(P<0.05),2组间意识恢复时间、定向力回复时间、离院时间、术中心率和血氧饱和度均无显著差异(P>0.05)。结论瑞芬太尼复合丙泊酚在门诊无痛胃镜中的麻醉时间和麻醉效果可靠,能充分满足需要。

Application of Remifentanil-propofol compound in painless gastroscope for outpatients

Objective To observe the anesthetic results of Remifentanil-Propofol Compound(RPC) in painless gastroscope for outpatients.Methods Thirty outpatients performed with painless gastroscope were randomly divided into two groups:the observation group(n=15) and the control group(n=15).In control group,the patients were induced to perform with intravenous injection with propofol per 2 mg·kg-1 at manual feeding speed of 2 mg·s-1,then intravenous injection with propofol per 2 mg·kg-1·h-1 by micro-pump after the outpatients were anaesthetized and tardily injected with more 20～30 mg propofol if necessary.In observation group,the patients were induced to perform with intravenous injection with propofol per 2 mg·kg-1 at manual feeding speed of 2 mg·s-1;after the outpatients were anaesthetized,tardily injected with remifentanil per 0.1 μg·kg-1,then intravenously micro-pumped with propofol per 2 mg·kg-1·h-1 plus remifentanil per 4.0 μg·kg-1·h-1.The anesthesia effection,the recovery time on orientation as well as the narcotic influence on the outpatients′ oxygen saturation and pulse.Results The anesthetization of the observation group is obviously more effective than the control group(P<0.05).There was no significant difference in recovery time of consciousness and orientation,discharge time and saturation of blood oxygen in two groups.Conclusion It is effective of combination of Remifentanil and Propofol Compound in painless gastroscope for outpatients.
