摘    要:目的探讨核素测定肾小球滤过率(GFR)检查方法对判断化疗后肾功能早期损害的价值。方法26例乳腺癌伴淋巴转移患者采用自身对照法在首次化疗前后行核素GFR测定,同时测定内皮素(ET)、尿β2-微球蛋白(β2-MG)、血尿素氮(BUN)、血肌酐(Cr)等肾功能变化指标进行对比观察。结果26例患者化疗后与化疗前比较GFR明显下降(t=6.489,P<0.01),内皮素上升(t=8.258,P<0.01),而尿β2-MG和血BUN、Cr指标变化不明显(P>0.05)。结论核素测定GFR能较早发现患者肾功能的损害,对于临床合理用药、观察和保护肾功能有一定的价值。

Abstract:Objective To explore value of determination of glomerular filtration rate(GFR) by nuclide for early impaired renal function in breast cancer after chemotherapy.Methods GFR in Twenty-six patients with breast cancer with metastases patients were determined with nuclide by self-contrast before and after the first chemotherapy.Endothelin(ET),urine β_2 microglobulin(β_2-MG),blood urea nitrogen(BUN),blood creatinine(Cr) which can reflect the changes of kindey function were simultaneously determined and compared contrastly.Results GFR in 26 patients after chemotherpy were obviously decreased compared with that before chemotherpy(t=6.489,P<0.01),endothelin was increased(t=8.258,P<0.01),and there were no significant difference in urine β_2-MG,blood BUN and Cr(P>0.05).Conclusion Determination of GFR by nuclide can detect early renal function damage.It is a certain useful value in clinical rational administration,clinical observation,protection of kidney function.
