重症肺炎患者细胞免疫功能研究

目的探讨重症肺炎患者的细胞免疫功能变化。方法筛选老年重症肺炎患者50例(老年重症肺炎组)、老年非重症肺炎患者31例(老年肺炎非重症组)、非老年重症肺炎患者12例(重症肺炎非老年组)、非老年非重症肺炎患者21例(非老年非重症肺炎组),流式细胞术检测患者外周血T细胞亚群(CD4+、CD8+、CD4+/CD8+)及自然杀伤(NK)细胞的活性,并用双抗体夹心法检测各组白细胞介素-6(IL-6)、白细胞介素-8(IL-8)及肿瘤坏死因子-α(TNF-α)水平。结果老年重症肺炎组患者外周血CD4+、CD8+、CD4+/CD8+及NK细胞活性均低于老年肺炎非重症组、非老年非重症肺炎组和重症肺炎非老年组(P<0.05,P<0.01);重症肺炎非老年组患者外周血CD4+、CD8+、CD4+/CD8+及NK细胞活性均低于非老年非重症肺炎组(P<0.05);老年肺炎非重症组患者与非老年非重症肺炎组患者外周血CD4+、CD8+、CD4+/CD8+及NK细胞活性比较差别均无统计学意义(P>0.05)。老年重症肺炎组患者IL-6、IL-8、TNF-α均高于老年肺炎非重症组、重症肺炎非老年组和非老年非重症肺炎组(P<0.05,P<0.01);重症肺炎非老年组和老年肺炎非重症组患者IL-6、IL-8、TNF-α均高于非老年非重症肺炎组(P<0.05)。结论重症肺炎患者细胞免疫功能较非重症肺炎患者下降,其中老年重症肺炎患者细胞免疫功能较中青年重症肺炎患者下降更明显。免疫功能减退可能是老年患者发生重症肺炎及疗程延长的原因之一。
