单纯性肥胖儿童胰岛素抵抗与游离脂肪酸的关系

摘    要:目的探讨单纯性肥胖儿童血清游离脂肪酸(FFA)与胰岛素抵抗(IR)的相关关系。方法选择50例单纯性肥胖儿童(单纯性肥胖组)和50例正常儿童(对照组),测定各组空腹血清FFA、空腹血糖(FPG)、空腹胰岛素(FINS)和血脂水平,并计算胰岛素抵抗指数(HOMA-IR)值。结果单纯性肥胖组中FFA、HOMA-IR、总胆固醇(TC)和甘油三酯(TG)高于对照组(P<0.05);单纯性肥胖组血清FPG与对照组比较无显著性差异(P>0.05)。在对照组和单纯肥胖组中,血浆FFA与体质量指数(BM I)、FPG、FINS、HOMA-IR、TG和TC呈正相关(P<0.05)。结论单纯性肥胖儿童存在IR,血清FFA与肥胖儿童的BM I、TG、TC及IR密切相关,FFA可能参与了IR的病理生理机制。

Abstract:Objective To probe the relationship between insulin resistance(IR) and free fatty acids(FFA) in children with simple obesity.Methods Fifty children with simple obesity(simple obesity group) and 50 cases of healthy children(control group) were collected from July 2005 to July 2007.Values of fasting serum FFA,fasting blood-glucose(FPG),fasting insulin(FINS),and blood-lipid level in both groups were surveyed.Insulin resistance index of each group was calculated.Results There were significantly higher FFA,homeostasis model assessmentes of insulin resistance(HOMA-IR),triglyceride(TG) and total cholesterol(TC) in the simple obesity group than those in control group(P<0.05).The level of FPG in the simple obesity group showed no signigicant difference compared with that of the control group(P>0.05).There were statistically positive correlations between FFA and body mass index(BM I),FPG,FINS,HOMA-IR,TG,TC(P<0.05).Conclusion There is insulin resistance in children with simple obesity.Serum FFA concentration is closely correlated with BM I,FPG,FINS,HOMA-IR,TG,TC,and insulin resistance.The data suggest a possible role of FFA in pathophysiology of insulin resistance.
