沙丁胺醇雾化吸入治疗小儿支气管哮喘急性发作疗效分析

摘    要:目的探讨雾化吸入沙丁胺醇治疗小儿支气管哮喘急性发作的临床疗效。方法86例支气管哮喘急性发作患儿,分为对照组40例,观察组46例。2组患儿均予抗感染、静脉滴注氨茶碱、口服酮替芬及止咳化痰等对症治疗,观察组在对症治疗基础上雾化吸入0.5 g.L-1沙丁胺醇悬液,观察治疗前后临床症状和肺功能的变化。结果观察组总有效率为91.3%(42/46),对照组总有效率为67.5%(27/40),观察组高于对照组,差别有统计学意义(P<0.01);治疗前观察组的第1秒用力呼气容积(FEV1)、用力呼出50%肺活量时呼气流量(FEF50%)、呼气峰流量(PEF)及用力肺活量(FVC)与对照组比较差别无统计学意义(P>0.05),治疗后观察组的各项指标明显提高,与治疗前比较差别有统计学意义(P<0.05),对照组改善不明显,与治疗前比较差异无统计学意义(P>0.05),治疗后观察组患儿的FEV1、FEF50%、PEF及FVC均高于对照组,差别有统计学意义(P<0.05,P<0.01)。结论沙丁胺醇雾化吸入可迅速缓解哮喘急性发作,改善肺功能,值得临床推广。

Abstract:Objective To investigate the clinical effects of atomization inhalation with salbutamol to treat acute episode of bronchial asthma on child. Methods 86 cases with acute episode of bronchial asthma were divided into control group( n = 40) and observation group(n = 46 ). All patients were treated by symptomatic treatment which including anti-infection, intravenous drip aminofilina,oral ketotifen and relieve cough and reduce sputum. Furthermore,the patients in observation group were given salbutamol 0. 5 g · L^-1 by atomization inhalation based on the symptomatic treatment. Then the changes of clinical symptoms and pulmonary function were observed before and after treatment. Results The total effective rate in observation group(91.3% )was higher than that in control group (67.5%) (P 〈 0. O1 ) . There were no difference in forced expiratory volume in one second (FEV1), maximal expiratory flow in 50% vital capacity (FEF50%), peak expiratory flow (PEF)and forced breathing capacity (FVC) between two groups before treatment( P 〉0.05 ). But the results of all the indexed in observation group were higher than those in control group after treatment ( P 〈 0.05, P 〈 0. O1 ). After treatment all the indexed in observation group were higher than before treatment( P 〈 O. 05 ), but there was no obviously improvement in control group ( P 〉 0.05 ). Conclusion The clinical symptom and pulmonary function of acute episode of bronchial asthma treated could be relieved by atomization inhalation with salbutamol, and it is worthy to be popularized.
