血浆血管内皮生长因子、尿微量白蛋白及β_2-微球蛋白与过敏性紫癜患儿早期肾损伤的相关性

摘    要:目的探讨血浆血管内皮生长因子(VEGF)、尿微量白蛋白(MA)及β2-微球蛋白(β2-MG)与过敏性紫癜(HSP)早期肾损害的相关性。方法73例HSP患儿分为皮肤型组23例、混合型组28例和紫癜性肾炎(HSPN)型组22例;对照组23例为健康体检者;治疗后组25例为治疗2周后临床无阳性症状、体征,出院前复查尿常规及24 h尿蛋白定量正常者。采用酶联免疫吸附测定-ABC法(ABC-ELISA)测定血浆VEGF,采用放射免疫分析法测定尿MA及β2-MG。结果HSPN型组血浆VEGF、尿MA及β2-MG水平明显高于皮肤型组和混合型组(P<0.05,P<0.01),治疗后组血浆VEGF、尿MA和β2-MG明显高于对照组(P<0.05);HSP患儿VEGF和MA呈正相关(r=0.444,P<0.05);VEGF和β2-MG呈正相关(r=0.275,P<0.05)。结论血浆VEGF、尿MA及β2-MG参与了HSPN的发病过程,是早期HSP并发肾损伤的敏感指标。

Abstract:Objective To explore the correlation of plasma vascular endothelial growth factor (VEGF) , urine microalbumin (MA) and β2--microglobulin (β2-MG) with early renal injury in children with Henoch-Schenlein purpura (HSP). Methods Seventy-three cases of HSP were divided into skin type group (23 cases) , mixed pattern group (28 cases) and Henoch-Schonlein purpura nephritis (HSPN) group ( 22 cases). Twenty-three of control group were healthy children. Twenty-five cases of post-treatment group had negative symptoms,normal urine analysis result and 24 h urine protein after 2 weeks treatment. Plasma VEGF was detected with enzyme-linked immunosorbent assay, Urine MA and β2-MG were detected with radioimmunoassay. Results The levels of plasma VEGF, urine MA and β2-MG in HSPN group were higher than these in skin type group and mixed pattern group ( P 〈 0.05, P 〈 0.01 ), and these in post-treatment group were higher than these in control group ( P 〈 0.05 ). It was positive correlation between VEGF and MA( r = 0.444, P 〈 0.05 ), it was positive correlation between VEGF and β2-MG( r = 0. 275, P 〈 0.05). Conclusion VEGF, MA and β2-MG may involve in the development of HSPN, they were sensitive marker of renal damage in early stage of HSP patients.
