颈动脉内膜剥脱术治疗颈动脉狭窄126例分析

目的总结颈动脉内膜剥脱术（Carotid endarterectomy，CEA）治疗重度颈动脉狭窄的临床经验。方法回顾性分析2007年1月-2011年2月在我科行CEA的126例患者临床资料，122例均是初次手术，4例为双侧狭窄对侧治疗后再次手术；108例术前有短暂性脑缺血发作（transient ischemic attack，TIA），17例术前曾患脑梗死，1例术前无症状（狭窄率〉90％）。所有患者术前经选择性颈动脉造影证实狭窄率70％～99％。其中35例患者术前经冠状动脉造影证实合并冠心病，6例同期行冠状动脉搭桥术。结果围术期1例患者因脑出血死亡。其余术前有临床症状的患者术后有较明显改善，TIA消失。术后随访2～24个月，1例在随访期间因心脏病死亡，其余患者病情稳定。结论CEA是治疗颈内动脉重度狭窄主要的有效方法。
