腹腔镜下输卵管修复术联合防粘连剂对妊娠率的影响因素分析

目的探讨影响腹腔镜下榆卵管修复术中联合防粘连剂对输卵管性不孕患者术后妊娠率的各种影响因素。方法对我院因输卵管性不孕行腹腔镜下输卵管修复术联合防粘连剂治疗的120例患者进行随访，分析影响输卵管不孕患者术后妊娠的因素。结果120例患者术后共妊娠37例，妊娠率为30．8％，其中活产率为81．1％（30／37），自然流产率为5．4％（2／37），异位妊娠率为13．5％（5／37）。输卵管盆腔病变重度、中度、轻度的妊娠率分别为20．0％、34．5％和58．3％，差异有统计学意义（χ^2=7．372，P〈0．05）。不孕类型、不孕时间、年龄、既往腹腔镜手术史、术前造影情况、术中亚甲蓝通畅情况、术中使用不同的防粘连剂等对妊娠率的影响比较差异无统计学意义。结论输卵管的病变程度可能是影响术后妊娠的主要因素之一。

INFLUENCE FACTORS OF LAPAROSCOPIC TUBAL REPAIR AND AGENT ON PREGNANCY RATE

Objective To investigate the factors of laparoscopic tubal repair and antiblocking agent on af- fecting the pregnancy rate. Methods In author＇s hospital from August 2008 to December 2011, 120 pa- tients who carried out the therapy of laparoscopic tubal repair and antiblocking agent for tubal infertility were followed up, and the influence factors of tubal infertility patients after pregnancy were analyze. Resuits After operation, 37/120 cases of pregnancy, the pregnancy rate was 30.8%, of which the live birth rate was 81.1% （30/37）, spontaneous abortion rate was 5.4% （2/37）, ectopic pregnancy rate was 13.5% （5/37）. The pregnancy rates were separately 20o/00, 34. 5% and 58.3% for patients with severe, moderate, mild of the tubal pelvic lesions, the difference was statistically significant （χ^2 = 7. 372,P〈0.05）. The infertility type, infertility duration, age, previous history of laparoscopic operation, preoperative angiography, unobstructed condition of intraoperative methylene blue, different use of antiblocking agent during operation, which had no significant difference to pregnancy rate. Conclusion Lesion severity of fallopian tube may be the main factor influencing postoperative pregnancy.
