开放的后入路无张力疝修补手术的临床应用

摘    要:目的探讨使用单层网片进行开放的后入路无张力疝修补手术的可行性、具体操作方法和临床疗效。方法对2006年7月—2008年8月间46例应用单层平面网片进行的开放的后入路无张力疝修补手术的病例资料进行分析讨论。结果平均手术时间(40±15)min。术后患者伤口疼痛轻微,均未使用镇痛药。术后6-24 h下床活动,无切口积血、切口感染及腹股沟区异物感等,随访无复发情况。结论使用单层聚丙烯网片对成人腹股沟疝进行后入路的无张力疝修补(Kugel手术)安全有效,术后恢复快,疗效满意,具有推广应用价值。

Abstract:Objective To evaluate the feasibility and specific methods of operation and clinical effect of tension-free hernioplasty for inguinal hernia through open posterior approach by using monolayer mesh patch. Methods Data of 46 cases with inguinal hernia who underwent tension-free hemioplasty through open posterior approach by using monolayer mesh patch were analyzed from July 2006 to August 2008. Results The average operative time was (40 ± 15)min. Patients suffered light postoperative pain and had no use of analgesics. The patients could leave bed 6 - 24 hours postoperatively and had no complications such as incision hematocele, incision infection and foreign body sensation in inguinal region. No recurrence occurred during follow up. Conclusion It is safe, effective and has quick postoperative recovery for the use of monolayer polypropylene mesh on adult inguinal hernia with posterior approach for tension-free hernioplasty. The recent curative effect is satisfactory and it has application value.
