血浆脑钠素、内皮素测定在慢性肺心病患者中的应用

目的 探讨慢性肺心病患者血浆脑钠素（brain natriuretic peptide,BNP）、内皮素（endothelin,ET）的测定及其意义。方法 对80例肺心病患者及30例正常受试者采用放射免疫分析法测定血浆BNP、ET,同时应用超声检测收缩期三尖瓣返流压力阶差（the tricuspid valve regurgitation pressure gradientbiggest,TRPG）估测肺动脉压（pulmonary artery pressure,PAP）。结果 肺心病患者血浆BNP、ET均显著高于正常对照组（P〈0.01）;肺动脉压力重度升高时,BNP水平高于轻、中度升高组,组间存在着显著性差异（P〈0.01）;且肺心病患者BNP和ET间呈正相关（P〈0.01）。结论 血浆BNP和ET共同参与了肺心病的病理生理过程,它们用于肺心病患者的检测具有重要的临床应用价值。

Determination and significance of plasma brain natriuretic peptide and endothelin in patients with chronic cor pulmonale

Objective To study the plasma level of brain natriuretic peptide(BNP) and endothelin(ET) and its significance in patients with chronic cor pulmonale.Methods The plasma level of BNP and ET were measured in 80 patients with chronic cor pulmonale and 30 healthy persons by using radioimmunoassay.The tricuspid valve regurgitation pressure gradient biggest(TRPG) in calculating pulmonary artery pressure was used in control group.Results The levels of BNP and ET for chronic cor pulmonale group were both higher than that in the normal control group(P<0.01).The plasma level of BNP was positively related to the plasma level of ET(P<0.01),and had some relationship with the pulmonary artery pressure according to TRPG.Conclusion The results suggest that BNP and ET are involved in the pathophysiological process of chronic cor pulmonale.The plasma BNP and ET detections in patients with cor pulmonale have important clinical practical value.
