外周血干细胞的采集与糖尿病足的治疗

目的研究观察自体外周血干细胞移植治疗糖尿病足的临床疗效。方法按照操作规范和病人的实际情况采集外周血干细胞，对3例自愿接受自体PBSC移植的糖尿病足患者施行十细胞移植各1次，全部患者均给予重组人粒细胞集落刺激因子200～500μg／d，皮下注射，行PBSC动员5d，然后用MCS＋ED血细胞分离机采集外周干细胞。将干细胞悬液按3cm×3cm距离进行患侧肢体移植术，术后第1天至3个月定期观察各项指标综合评估。结果PBSC移植后3个月进行评价，3例患者疼痛明显缓解，皮温升高，下肢凉感消失，感染得到控制，溃疡或足趾坏疽明显好转或愈合，足趾色由暗紫转为正常，肿胀消失．经血管造影显示均有丰富的毛细血管产生。无一例患者行截肢术；移植后所有患者均未出现并发症和明显不良反应。结论自体PBSC移植治疗糖尿病下肢缺血性血管病是一种安全、有效的手段，可使一部分患者免除截肢，改善其生活质量。

Collection of Peripheral Blood Stem Cells and Treatment of Diabetic Foot

Objective To observe the clinical effect of autologous peripheral blood stem cells in the treatment of diabetic foot. Methods Stem cells were transplanted into three patients of diabetic foot who volunteered to receive autologous peripheral blood stem cells. Various index were observed from the first day to three months after operation. Results After three months of PBSC transplantation, pain of patients relieved, skin temperature increased, infection was controlled, ulcer or toe grangrene was improved or healed ,the toe colour became normal, swelling disappeared. No amputation was operated in one patient. All patients had no complications and clear side effects. Conclusion Autologous PBSC transplantation is an effective and safe method in the treatment of diabetic ischemic angiopathy of lower limb. It can exempt amputation of some patients and improve the quality of life in diabetic patients.
